Diabetes Center

The Diabetes Center offers an outpatient diabetes self-management education program. This is a comprehensive program, which provides patients an opportunity to acquire the skills required to control their diabetes and prevent the associated complications.

Accredited by the American Diabetes Association and the Michigan Department of Community Health, we are staffed by an RN and RD with CDE (Certified Diabetes Educator) credentials.

This program is covered by most insurances

Educational services are also offered at our Main Street Internal Medicine and Pediatrics location on Main Street in Fremont, Grant Medical Center, Hesperia Medical Center, and Pine Medical Group in Fremont and Newaygo. We are affiliated with TIPDON, which can be reached at:
